Before you proceed, kindly read the following carefully

Before filling out registration form, prepare make sure you have the ORIGINAL birth certificate of the student as this will be used to confirm his registration

Please note that ONLY students who are currently in Grade 3, Grade 4 and Grade 5 (primary school) are eligible to apply and must be aged between 8 to 12 years

It is important that you review the information you enter and correct any errors before clicking "Submit" button

By filling out this form, to best of my knowledge, all information is correct and I am responsible for any inaccuracy or incomplete information, as well as for all files uploaded and their quality

Remember! on the day of the assessment, please bring a copy of student's birth certificate
